FOR ADMISSIONS

Lists of required documents for applicants by cohort:

 

Born in Quebec

  1. Copy of Birth Certificate
  2. Copy of Medicare Card
  3. Copy of Social Insurance Number
  4. Copy of Proof of High School Completion
    1. High school diploma (for DEC programs)
    2. High school diploma, transcript or CV (for AEC programs)

 

Permanent Resident or Citizen

  1. Copy of Birth certificate (if the birth certificate was not written in the English or French language, you must provide both the original and translated copy)
  2. Copy of Medicare Card
  3. Copy of Social Insurance Number
  4. Copy of Permanent Residency Card or Citizenship Card / Certificate
  5. Copy of CSQ (Certificate of Selection to Quebec)
  6. Copy of Proof of High School (same proofs as above for DEC or AEC programs)
*IF YOU DO NOT HAVE A CSQ:

1. If your spouse was born in Quebec, you must provide a copy of your spouse’s birth certificate and your marriage certificate.

 

OR

2. If your spouse was NOT born in Quebec, you must provide the following: 

  • Lease or property tax bill

AND

  • 2 utility bills (phone, hydro, credit card…)

 

If the applicant lives with their parents

You must provide:

  • A copy of one parent’s medicare card

AND

  • A signed letter from the same parent

*The letter from a parent should clearly state that the applicant is the child’s parent and has been living with them in Quebec since the date they arrived (the parent must clearly indicate the date of arrival in Quebec.)
